I have some exciting news to announce in the medium-near future about a possible cure for chronic Lyme disease. It is a based on a special delivery mechanism for antibiotics, using nanotechnology that allows the blood-brain-barrier to be penetrated so it's not something we can sell in a webstore, but we may partner with a production lab and a MD in the not so far future. Never pick off a scab, if you remove it prematurely, it can leave a scar. Scabs must be left until they are "ripe" and fall of by themselves.

You should wait longer (at least six weeks from now) before you start microneedlling the scar because it may still improve by itself, especially because you removed the scabs prematurely.

Milia consist of keratin - the skin's hardest protein that makes the outer skin layer so tough. Hooves and nails are also made of keratin.

You can pierce a milium with the single needle (start with making one or two pricks into it - it should be enough) and extracting the entrapped keratin by very gently pushing it out with two paperclips.

Salicylic acid has a keratolytic effect (softens keratin) - it would also be useful.

Not using anti-inflammatory substances applies only to those substances with a STRONG anti-inflammatory effect, applied immediately after dermarolling (especially after dermarolling with long needles). Some hours later, it is already OK.

Lots of substances have a very mild anti-inflammatory effect (for example certain oils) but that is not a problem. Avoid creams with steroids, all non-steroidal anti-inflammatory creams, creams indicated to relief itchiness and irritation, creams with a high percentage of aloe vera etc. and you'll be fine.

You can use a 0.1% Tretinoin cream on your face if your skin can handle it. No reason for concern. However, lower dosages are still very effective (equally effective, usually) and the more is not always the better with Tretinoin. The higher the percentage, the greater the irritation (redness, dryness). Usually, the initial irritation goes away once the skin gets used to Tretinoin but in some cases it prevails.

The extremely rare danger of overdosing on vit. A results from the absolutely gigantic and prolonged oral intake, especially from highly dosed supplements or from eating the organs of animals that contain large amounts of vit. A (as it sometimes happened during polar expeditions, eating polar bear liver, cod liver etc.)

Applying Tretinoin right after dermaneedling should be done only in special cases anyway. Below, I copied my forum postings from other threads concerning that subject:

For scars or stretch marks, I always recommend long needles (1 mm or longer). The frequency of rolling with a 1.5 mm size is every three weeks. So you apply Tretinoin right after microneedling every three weeks.  You can also spread out your rolling session and roll for example one cheek (apply Tretinoin), some days later another cheek (apply Tretinoin) etc. The same with stretch marks or surgical scars (needle every three weeks and apply Tretinoin right after).

In the meantime, you can apply Tretinoin without dermarolling.

I have never recommended rolling every day really extensive areas and applying Tretinoin right after. Rolling three times a week your face with a 0.5 mm roller and applying Tretinoin is OK but the skin may get irritated.

Special concerns must be taken during pregnancy and breastfeeding but during those times, special concern must be taken about everything, even about a glass of wine and the best approach is to temporarily quit everything.

On this forum, I actually do recommend Tretinoin (A-Ret) application on scars right after microneedling:

"I do not recommend using A-Ret right after dermarolling because it is acidic and it stings when applied to the rolled skin but in some special cases such as stubborn scars and stretch marks, I do recommend it".


In our dermarolling instructions, I mention it as well:

"If you apply A-Ret right after rolling (recommended with stubborn scars and stretch marks or wrinkles) your skin will get even more red and it will sting. It may stay red for several days."
 
The problem is that significantly many customers have quite some strong reactions after Tretinoin application (redness, dryness) and this reaction will be much more sever if Tretinoin is applied right after microneedling so I have to be careful, recommending it.

Your skin seems to take Tretinoin application very well, you are lucky.

One has to moisturize a lot when using Tretinoin. For example, wet a cotton pad with tap water, add some almond oil and apply it to your face to offset the dryness triggered by Tretinoin.

It is not necessary to get scabs, neither is it unusual not to get them. The only important thing is that if you do get them, do not remove them forcefully because premature removal of a scab could leave a scar. Let them fall off naturally when they are "ready".

Yes, you can stamp. It will get red but in order to improve scars, you have to suffer a little..

Do not apply anti-inflammatory substances immediately after mcironeedling. Wait a day, preferably even more.

You can actually squeeze a little Infadolan into a small cup and put it on low in the microwave to warm it up (do not put Infadolan tube into the microwave!).

Alternatively, use the au-bain-marie method.

There is currently no method that can remove surgical scars but you can manage to make the scar less visible and improve its texture and color.
