I am not completely sure what you mean. Do you have hyperpigmentation now in the scars (brownish red color) or did they turn really black and they look like blackheads (that would be very strange and I have never heard about this). Would it be possible for you to send a photo to sarah@owndoc.com?

Did you needle the scars during those five months? How many times have you needled in total? Did it happen to all the needled scars or just some of them?

You should start applying the A-Ret on one scar only to see what happens.

Try a 1.5 mm dermastamp. Start slowly and if it heals well, stamp more densely. Nose skin is a bit unpredictable because there is basically just cartilage below. If the scar were for example on the forehead, microneedling would be more likely to improve it however it is worth trying on the nose as well.

You have to roll more than once in each direction. Roll at least 8 times in each direction.

The reason why a 1.5 mm ONE LINER dermaroller is less painful that a 1.5 mm dermastamp is because there are less needles in the skin in one go. A one liner roller has only 24 needles, whereas a dermastamp has 35 needles.

A regular 1.5 mm roller has 192 needles.

As for your question about the length used, I think I started with 1.0 and used it all over my chest about 2 to 3 times per week intitially. If your scars are less "deep" you could use the .5 length, but at least 2 of my scars were so severe that I could literally feel that they were harder than the rest of my skin, if that makes sense... the white scars felt really dense, so I was aggressive in trying to break up that scar tissue. This site has a very good section on what length roller to use for different areas of the body. Each area and scar are different, but the administrator of this site tries to answer as many questions as possible, either in a response directly or by putting the information to answer your question on this site. You can use any numbing cream that is sold OTC (do not use a liquid numbing agent because it doesn't get absorbed by intact skin, buy a cream) but do not use it on extensive areas during one session. For example, do not apply it all over you stomach + all over your butt in one session.  You can use it again the day after.  If you have very extensive areas to roll and you want to use a numbing cream on all of them, spread it out to several days. Wipe the numbing cream off before you roll.

   

  There is usually no need for a numbing cream when you roll for stretch marks. It is painful but it is bearable. You get used to it.

   

  I can roll anywhere on my body or face without a numbing cream with the exception of the above the lip area and I am not particularly tough person when it comes to pain.

I think in your case the single needle is not suitable. I recommend a 0.5 mm regular dermaroller. Roll about three times a week.

If the spots are acne-related, buy our 0.025% Tretinoin cream. It should keep it under control but Tretinoin initially dries out the skin and can be irritating so do not apply it every day and use just a little.

No, the saline solution method is not a temporary solution. The saline solution (also called a physiological solution) consists of water and a little NaCl (table salt) and it is normally present in the body. It doesn't serve as a filler because the "bump" that forms when you inject it absorbed within hours. You just inject water under the scar, that is all.

It serves the same purpose as the suction method - to stretch out the fibrotic bands that tether the scar down and make it indented and to trigger collagen production.  It is injected straight after a subcision or needling.

By the way, Hyaluronic acid (Restylane) is also normally present in the body and the fillers from hyaluronic acid would also last mere hours. That is why the hyaluronic acid in the fillers is crosslinked, which makes it much longer lasting.

Aloe Vera is a strong anti-inflammatory and should not be applied immediately after rolling/stamping. Some hours later is already OK:

You cannot reach all the stretch marks on the butt with the single needles/dermastamp but needle all you can reach.

Stretch marks extend through the epidermis and reach the deep part of the dermis (called the reticular dermis). The thickness of the skin (epidermis + dermis) is not the same throughout the body. It varies, depending on the location on the body and it also varies individually. The thinnest skin is on the upper eyelids (ca. 0.5 mm), the thickest on the footsoles (ca. 4 mm). For the rest it is about 1.3 mm to 1.8 mm thick (the skin on the back can be 2 mm, especially in men).

Below the skin, there is a thick layer of fat, then muscle and then bone.

A 1.5 mm roller penetrates about 1.3 mm into the skin, which is very deep into the dermis and it is sufficient for stretch marks.

You can use a 2 mm dermaroller but it will be more painful and likely it will have no extra effect comparing to the 1.5 mm size. As a beginner, you should always start with shorter needles and when they get blunt, you can buy longer needles.

Currently, there is no method (including dermarolling) that can remove stretch marks. Currently, stretch marks can only be improved:

It is certainly realistic to get improvement in 3-5 treatments (with  >1 mm size) but it doesn't mean the stretch marks will be gone and it doesn't mean that everybody will get improvement after only 3-5 sessions. The clinics

just recommend 3-5 sessions and depending on the results, you will need more.

We have been always advocating to use the combination of a 1.5 mm roller + the single needles for stretch marks. The single needles penetrate deeper than a dermaroller and they can target the marks densely and individually.

Single needling is laborious but very effective in combination with dermarolling.

Disinfecting  the dermaneedling instrument after dermarolling is more important than disinfecting the skin before dermarolling. The most important part of the disinfection is to clean the dermarolling instrument after dermarolling with dishwashing detergent and warm water and rinse it under a very strong stream of water to remove blood and skin debris from the needles. Then soak it in Chloramine-T or Ethanol.

Do not use the single needles on your inner arms. From our experience, the skin is too thin there and you'll get bruises.  Use only the roller there.

Wait two more weeks and then start stamping your scar about every 20 days.

The missing tissue makes the scar intended and hopefully, stamping will fill in or at least partially fill in the scar. In addition, the scar may be tethered to the underling structures, which makes it also intended. You could improvise the suction method, which releases the fibrotic tissue that pulls the scar down:

Small creases or any other unevenness in color or texture is totally normal after microneedling. The skin is being healed and renewed and until the process is completely finished, the skin can look different from normal.
